This position involves providing skilled nursing care and supportive services to patients in their homes. The successful candidate will work under the direction of a Registered Nurse and will be responsible for delivering high-quality patient care while maintaining a safe and comfortable environment.
Key Responsibilities:
- Provide skilled nursing care to patients in their homes, including administering medications, performing treatments, and providing education on self-care techniques.
- Assist patients with transfer and ambulation, using good body mechanics to ensure patient safety.
- Communicate effectively with patients, families, and healthcare team members to ensure seamless care coordination.
- Document patient progress and care provided, according to company policy.
- Participate in on-going patient assessment, including patient response to treatment, under the supervision of a Registered Nurse.
- Develop and implement a realistic, goal-oriented plan of care, in collaboration with the patient and healthcare team.
Requirements:
- Current registration/licensure in the state(s) in which you will practice and in good standing with the Board.
- Good communication skills, both verbal and written.
- Demonstrated problem-solving and decision-making abilities.
- Ability to develop and maintain rapport with patients and families.
Work Environment:
- Primarily medium in nature, with exerting up to 35 pounds of force to lift, carry, push, pull, or otherwise move objects, including the human body.
- Exposure to disagreeable conditions, including odors, infection, illness, and physical contact from disruptive patients.
