Job Summary: We are seeking a Licensed Practical Nurse (LPN) to join our care team. As an LPN, you will provide direct patient care, administer medications, and work collaboratively with our interdisciplinary team to ensure the best possible outcomes for our residents.

Responsibilities:

  • Provide direct patient care, including administering medications, wound care, and vital sign monitoring
  • Develop and implement individualized care plans in collaboration with the interdisciplinary team
  • Conduct thorough assessments of residents' physical and emotional needs
  • Communicate effectively with residents, families, and healthcare team members
  • Participate in quality improvement initiatives and contribute to a culture of excellence

Requirements:

  • Current LPN licensure in the state of Minnesota
  • Excellent communication and interpersonal skills
  • Ability to work collaboratively as part of a team
  • Flexibility to work a variety of shifts, including days, evenings, and overnights
