Position Overview:

This role involves assessing, planning, implementing, and evaluating nursing care for patients in alignment with established organizational policies and professional nursing standards. The individual in this position is responsible for the quality of nursing services provided by themselves and any personnel under their supervision.

Key Responsibilities:

  1. Patient Assessment: Conduct comprehensive evaluations of patients' physical, psychological, social, spiritual, educational, developmental, cultural, and discharge planning needs. Review patient history and collaborate with families to ensure timely completion of assessments.
  2. Care Planning: Develop individualized care plans, including discharge strategies, based on assessment data and input from patients, families, and the healthcare team. Establish realistic and measurable goals tailored to the patient's specific needs.
  3. Implementation of Care: Execute nursing interventions and document care provided, ensuring adherence to best practices and policies.
  4. Collaboration: Work closely with interdisciplinary teams to coordinate patient care and communicate significant findings to physicians and other healthcare providers.
  5. Continuous Evaluation: Regularly reassess patient conditions and modify care plans as necessary to meet evolving patient needs.
