Job Overview

We are seeking an experienced and compassionate Patient Care Professional to join our multidisciplinary team at Medi Home Hospice in Centre and surrounding counties, PA.

About the Role

This full-time position involves working under the general direction of the Director of Nursing or Hospice Administrator, providing support to patients and their families to maximize comfort and health.

The successful candidate will be responsible for:

  • Planning and delivering patient care
  • Adhering to policies and procedures
  • Identifying patient/family needs and communicating with physicians
  • Providing supportive care in accordance with attending physician's orders
  • Participating in on-call rotations as needed
Requirements and Qualifications

To be considered for this role, you must have:

  • A successful completion of an accredited/certified registered nursing program
  • A current license as a registered nurse in the state of PA
  • A minimum of one-year experience in direct patient care
  • Hospice nursing experience is strongly preferred
  • Reliable transportation and company-required auto liability insurance*
