As an Inpatient Case Manager, you will play a crucial role in coordinating care for patients with complex medical needs. You will work closely with our interdisciplinary care team to ensure that patients receive comprehensive and compassionate care throughout their hospital stay. With a strong focus on patient-centered care, you will be responsible for identifying potential health risks and developing strategies to mitigate them.

We offer a competitive salary of $92,575.51 - $115,719.39 annually, based on experience. In addition to a rewarding salary, we also offer a range of benefits, including medical, dental, and vision insurance, 403(b) retirement savings plans, flexible spending accounts, and paid time off. To be successful in this role, you will need to have a strong background in nursing, with at least three years of experience in public health nursing, acute care, or case management.
