Optum is dedicated to assisting medical practices in transitioning to advanced team-based care models that cater to the needs of individual patients and broader populations. A crucial aspect of this transition involves evolving the business model from primarily Fee-For-Service (FFS) to a combination of FFS and Value-Based Care (VBC). This shift emphasizes quality and resource management over sheer volume.
Value-Based Care hinges on meticulous documentation and coding to accurately reflect the burden of illness and showcase the quality of care provided to every patient within the population. This process must occur at least once annually. The Clinical Performance Nurse Practitioner (also referred to as the "Embedded NP" or ENP) plays a vital role in engaging patients where they are and ensuring that all care and documentation gaps are addressed promptly. This role may involve travel to various independent clinic sites within the Optum Care Network. Travel time is compensated, and travel expenses are reimbursed per mile. If overnight travel is necessary, schedules will be coordinated with the Nurse Practitioner, ensuring appropriate accommodations are arranged as needed.
The ideal candidates are those who prioritize patient care over volume. We seek clinicians who are willing to invest the time necessary to genuinely meet our patients' needs. Flexibility and comfort in diverse settings (including home visits) are essential. This position does not entail managing a dedicated panel of patients or ongoing therapeutic management of acute or chronic conditions. Potential care sites include:
- Primary care clinics affiliated with the OptumCare Network
- Various specialty clinics (specific locations will be determined based on clinician home base)
- Home visits and long-term care facilities
- Virtual consultations
Key Responsibilities:
- Identify and address quality care gaps
- Recognize and manage factors related to chronic conditions that may have gone unnoticed, collaborating with patients to enhance their health status
- Conduct and document necessary screening tests for chronic conditions or their complications
- Inform patients about available resources within the OptumCare Network
- Facilitate connections to care management or other services when needed

Required Qualifications:
- Advanced degree as a Nurse Practitioner (ARNP or DNP)
- Current and unrestricted medical practice license in Oregon State
- Current and unrestricted DEA certificate
- Valid driver's license and reliable vehicle
Preferred Qualifications:
- Two years of experience as a primary care clinician
- Experience with chart review and documentation in an Electronic Medical Record (EMR)
- Experience with geriatric patients in value-based care contracts, such as Medicare Advantage
Essential Skills:
- Comprehensive knowledge of medical practice. Ability to communicate clearly with patients, families, agencies, other providers, and staff regarding patient care
- Proficiency in Microsoft Office applications and similar tools, including Word, Excel, and PowerPoint
- Ability to communicate through language translation services (both in-person and remote)
Interpersonal Skills:
- Ability to foster positive interactions with patients, families, physicians, administrators, and colleagues to effectively care for patients
- Key competencies include: curiosity, communication, collaboration, relationship management, storytelling, and adaptability to change
