Job Summary:

The LVN Charge Nurse will play a vital role in ensuring the delivery of high-quality care to our residents. This includes assessing resident needs, developing and implementing individualized care plans, and collaborating with the healthcare team to achieve optimal outcomes.

Key Responsibilities:
  • Conduct regular rounds to assess resident condition and identify areas for improvement
  • Develop and implement individualized care plans in collaboration with the healthcare team
  • Administer medications as prescribed and monitor resident response
  • Collaborate with physicians to ensure accurate and timely documentation
  • Oversee and monitor the work of nursing aides to ensure high-quality care
  • Identify and report any significant changes in resident condition to the Director of Nurses and/or Administrator
  • Initiate and complete incident and accident reports as necessary
  • Discharge residents in accordance with facility policy
Requirements:
  • Active Licensed Vocational Nurse (LVN) license in the state of Texas
  • Experience working in a skilled nursing facility preferred
  • Ability to effectively communicate with residents, families, and healthcare team members
Physical Demands:

The physical demands of this role include:

  • Standing, sitting, walking, and balancing
  • Reaching, stooping, kneeling, crouching, and crawling
