Rehab Without Walls Neuro Rehabilitation is a revolutionary neuro rehabilitation program that moves individuals outside the walls of institutional settings and into their natural home and community environment through a continuum of care.
Our program is designed to significantly help people – who have primarily experienced a brain injury, spinal cord injury or stroke/CVA through accident or illness – regain functional capabilities.
By providing care in the home, community, workplace, or school, we promote greater health through the recovery process, all with industry-leading outcomes that dramatically improve functional skills and independence for patients.
Job SummaryWe are seeking an experienced Physical Therapist to join our team. As a Physical Therapist, you will be responsible for providing functional rehabilitation to patients in their natural environment, whether at home, school, work, or in the community.
You will work collaboratively with our interdisciplinary team to develop and implement treatment plans that address the unique needs and goals of each patient.
Responsibilities include:
- Performing evaluations and developing treatment plans
- Regularly re-assessing effectiveness of treatment plans and attending staffing meetings and family conferences
- Performing discharge evaluations and making appropriate recommendations for home and community safety
- Communicating patient's needs and progress to the treatment team, physician, person receiving services and family members
- Communicating with other disciplines to ensure collaboration, coordination of care and enhance patient outcomes
To be successful in this role, you will need:
- A minimum of a bachelor's degree in Physical Therapy from a college or university with an accredited Physical Therapy program
- A current, unrestricted license as a Physical Therapist by the state in which you are practicing
- Current Basic Life Support or Cardiopulmonary Resuscitation certification in accordance with state regulations
- A minimum of one year's work experience as a physical therapist preferred
- Demonstrates knowledge of rehabilitation techniques related to complex neurological injury
- Communicates effectively and professionally in verbal and written interactions
- Ability to lift 50 pounds and perform other physical demands of the job
