We are seeking a highly skilled and compassionate Registered Nurse to join our interdisciplinary team as a Hospice Care Professional. As a key member of our team, you will provide high-quality patient care, support, and guidance to patients and their families, while working collaboratively with our healthcare professionals to ensure seamless care delivery.

Key Responsibilities
  • Provide comprehensive patient assessments, develop and implement individualized care plans, and coordinate care with other healthcare professionals.
  • Deliver high-quality patient care, including symptom management, pain control, and emotional support, in accordance with our organization's standards and best practices.
  • Collaborate with our interdisciplinary team to ensure comprehensive care delivery, including medical, social, and spiritual support.
  • Communicate effectively with patients, families, and healthcare professionals to ensure seamless care delivery and address any concerns or issues.
  • Participate in ongoing education and training to stay current with best practices, regulations, and industry developments.
Requirements
  • Current Registered Nurse (RN) licensure in good standing in the state(s) of practice.
  • Minimum 1 year of experience in hospice care or a related field.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare professionals.
  • Strong assessment and problem-solving skills, with the ability to develop and implement individualized care plans.
  • Ability to work independently and as part of a team, with a strong commitment to delivering high-quality patient care.
