Key Responsibilities- Provide comprehensive patient assessments, develop and implement individualized care plans, and coordinate care with other healthcare professionals.
- Deliver high-quality patient care, including symptom management, pain control, and emotional support, in accordance with our organization's standards and best practices.
- Collaborate with our interdisciplinary team to ensure comprehensive care delivery, including medical, social, and spiritual support.
- Communicate effectively with patients, families, and healthcare professionals to ensure seamless care delivery and address any concerns or issues.
- Participate in ongoing education and training to stay current with best practices, regulations, and industry developments.
- Current Registered Nurse (RN) licensure in good standing in the state(s) of practice.
- Minimum 1 year of experience in hospice care or a related field.
- Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare professionals.
- Strong assessment and problem-solving skills, with the ability to develop and implement individualized care plans.
- Ability to work independently and as part of a team, with a strong commitment to delivering high-quality patient care.
