Job Overview

Position Summary

We provide primary care services and excel in managing complex medical conditions, ensuring strong relationships between healthcare providers and patients. Our collaborative approach involves a multidisciplinary team, including nurses, administrative staff, clinical pharmacists, care assistants, nutritionists, physician assistants, nurse practitioners, and physicians, all working together to deliver high-quality patient care.

Key Responsibilities:
1. Assist in creating and maintaining personalized, goal-oriented care plans while collaborating with other healthcare professionals to meet the needs of patients and caregivers.
2. Monitor, evaluate, document, and communicate patient responses to treatments and therapies, adjusting care priorities based on individual needs. Collect and report relevant data regarding patients' physical, psychological, and educational requirements.
3. Engage in departmental activities that foster positive outcomes, including committee participation, quality improvement initiatives, and the training of new staff.
4. Deliver a wide range of treatments and therapies, such as respiratory care, medication administration, monitoring of vital signs, and assisting physicians with procedures.
5. Support patients and families during their clinical journey, providing orientation and guidance throughout their experience.
6. Offer emotional support to patients and families, utilizing de-escalation techniques for those in distress and promoting coping strategies appropriate for their age.

Qualifications

Educational Background:
* Completion of an accredited practical nursing program is required.
Licensure/Certification:
* Must hold a valid Practical Nurse (LPN) license in North Carolina and possess BLS certification.
Experience:
* No prior experience is necessary.
