Job Summary:

This position is responsible for providing exceptional care and support to our residents at The Addison of McCullough Place. Under the direction and supervision of a Registered Nurse (RN), LPN/LVN, or Director of Health & Wellness, the successful candidate will be responsible for delivering high-quality care and services to our residents.

Key Responsibilities:

  • Take and record vital signs, including temperature, pulse, and blood pressure.
  • Measure and record height, weight, and fluid intake/output to ensure accurate tracking of resident health.
  • Recognize and report any abnormal changes in body functioning to a supervisor, ensuring prompt attention to resident needs.
  • Assist with medication reminders, bathing, grooming, dressing, escort service, and other activities of daily living to promote resident independence and dignity.
  • Transfer, position, and turn residents safely and efficiently to prevent falls and injuries.
  • Provide skin care and other nursing rehabilitation/restorative nursing services to promote resident well-being.
  • Contribute to the resident's assessment and plan of care, working collaboratively with the healthcare team to ensure comprehensive care.
  • Follow the schedule of resident needs set out by supervisor to ensure timely and effective care delivery.
  • Provide emotional and social support to residents, promoting a positive and respectful environment.
  • Respect and encourage the independence and dignity of residents, while maintaining confidentiality and adhering to all applicable laws and regulations.

Additional Responsibilities:

  • Inform supervisor of any resident issues or concerns, ensuring prompt attention to resident needs.
  • Attend all required training, in-service, and staff meetings to stay up-to-date on best practices and company policies.
  • Strive to maintain a safe working environment through the prevention of accidents, preservation of equipment, and achievement of safe working practices.
  • Maintain a positive and professional demeanor toward residents, visitors, families, and co-workers, promoting a positive work environment.
  • Adhere to all policies and procedures of The Addison of McCullough Place, ensuring compliance with company standards and regulations.
