Job Summary

We are seeking an experienced Clinical Care Coordinator to join our team at The Laurels of Athens. This is a unique opportunity for a skilled nurse to remain clinically involved in patient care without being a bedside nurse.

About the Role

The Clinical Care Coordinator will work closely with our interdisciplinary team to assess and coordinate resident care plans. Some key responsibilities include:

  • Completing comprehensive assessments and care plans within regulatory time frames.
  • Collaborating with healthcare professionals, including physicians, nurses, social services, therapy, dietary, and activity staff to discuss resident care needs.
  • Reviewing information from hospitals, consults, and outside agencies to inform assessment and care planning.
Responsibilities

The successful candidate will be responsible for:

  1. Conducting thorough physical assessments, interviews, and chart reviews to identify resident care needs.
  2. Developing and implementing individualized care plans that meet resident goals and preferences.
  3. Coordinating with healthcare providers to ensure timely and effective care delivery.
  4. Maintaining accurate and up-to-date records of resident care and progress.
Requirements

To be considered for this role, you must have:

  • A valid Registered Nurse (RN) or Licensed Practical Nurse (LPN) license.
  • Experience as an MDS Nurse, preferably in a skilled nursing setting.
  • Familiarity with the Resident Assessment Instrument (RAI) process and Prospective Payment System (PPS).

The Laurels of Athens offers a competitive salary range of $62,000 - $80,000 per year, based on experience, plus a comprehensive benefits package, including health insurance, 401K matching, paid time off, and paid holidays.
