Job Summary:

We are seeking a skilled Registered Nurse Case Manager to join our team at Interim HealthCare of New Hampshire Careers. As a key member of our healthcare team, you will be responsible for managing a defined caseload of patients, providing direct patient care, and delivering comprehensive nursing services to individuals in the community.

Responsibilities:

  • Manage a defined caseload of patients, providing direct patient care and comprehensive nursing services.
  • Develop and implement individualized care plans to meet the unique needs of each patient.
  • Collaborate with healthcare professionals to ensure seamless care coordination and communication.
  • Conduct regular patient assessments to identify and address any changes in condition or needs.
  • Provide education and support to patients and their families to promote health and wellness.

Requirements:

  • Valid nursing license in the state of New Hampshire.
  • 2+ years of experience in skilled care, preferably in a case management role.
  • 1+ year of experience in nursing supervision, preferably in a leadership role.
  • Recent wound care experience.
  • Ability to lift 50 pounds with no restrictions.
  • Current CPR certification.
  • Reliable transportation.
