Job Summary

Equus Workforce Solutions is seeking a skilled Licensed Vocational Nurse (LVN) to fill the role of Medical Case Manager. This position will be responsible for assessing injuries, implementing or recommending treatment, and coordinating care with participants, families, and healthcare providers.

Key Responsibilities
  • Assess participant injuries and develop treatment plans in collaboration with the treatment team
  • Act as liaison between participants, families, and healthcare providers to ensure seamless care coordination
  • Work closely with the Medical Director and Manager of Medical Services to ensure proper case coordination and implementation of treatment plans
  • Document all clinical and case management activities in accordance with program, company, and regulatory requirements
  • Manage participant and program records, ensuring compliance with regulatory requirements
  • Participate in on-call crisis rotation and provide services that respect each participant's cultural background and identity
  • Represent the company at conferences, job fairs, and community events as needed
  • Maintain confidentiality and adhere to the company's Health Insurance Portability Accountability Act (HIPAA) privacy program and Business Ethics and Compliance programs
Requirements
  • Licensed Vocational Nurse (LVN) preferred, with other licensed medical disciplines considered
  • Maintains unrestricted license to practice as a Licensed Vocational Nurse (LVN) or equivalent medical discipline in the state(s) to which assigned
  • Maintains current CPR certification
  • Demonstrates excellent organizational, time management, and computer skills
  • Demonstrates effective and professional interpersonal, verbal, and written communication skills
  • Able to work independently and as part of a team
