Key Responsibilities:

  • Oversee patient care and discharge planning activities
  • Support patients and their families in making informed treatment choices
  • Track patient length of stay and accelerate discharge when appropriate
  • Evaluate patient requirements and formulate personalized care strategies
  • Work alongside interdisciplinary teams to enhance care delivery

Qualifications:

  • Graduate of an accredited nursing program or possess a Master's in Social Work
  • Prior experience in case management within an acute care environment
  • Current RN or LMSW licensure
  • Preferred certification in Case Management
  • BLS certification is mandatory
