Job Title: RN Home Health Contract Position

Skyline Med Staff Home Health is seeking a skilled and dedicated Registered Nurse (RN) to join our team as a Home Health contract nurse in Lewisburg, West Virginia.

Job Summary:

We are looking for a compassionate and experienced RN to provide high-quality patient care in the comfort of their own homes. As a Home Health contract nurse, you will work closely with our interdisciplinary team to develop and implement personalized care plans that meet the unique needs of each patient.

Responsibilities:
  • Provide skilled nursing care to patients in their homes, including wound care, medication management, and education.
  • Conduct thorough assessments of patients' physical and emotional needs, and develop care plans to address these needs.
  • Collaborate with other healthcare professionals, including physicians, therapists, and social workers, to ensure comprehensive care.
  • Document patient progress and communicate with the healthcare team to ensure continuity of care.
  • Participate in ongoing education and training to stay up-to-date on best practices and industry developments.
Requirements:
  • Current RN licensure in the state of West Virginia.
  • Minimum 1 year of experience in home health or a related field.
  • Strong communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Flexibility to work a variety of shifts, including days, evenings, and weekends.
