- Providing outstanding patient care experiences aligned with individualized care plans.
- Recognizing the patient and family as the central unit of care.
- Conducting initial and ongoing spiritual evaluations of patients, families, and caregivers with a comprehensive approach to offer spiritual guidance, counseling, crisis intervention, and services tailored to patient and family objectives.
- Understanding how faith and beliefs assist patients and families in coping with their illness, prognosis, and experiences of grief and loss.
- Offering support to patients and families dealing with existential and spiritual distress, as well as spiritual concerns and overall well-being.
- Assessing the impact of illness on the ability to engage in religious or spiritual practices that enhance well-being.
- Identifying specific rituals or practices and facilitating opportunities for expression and engagement.
- Connecting patients with spiritual, religious, or faith communities as necessary.
- Managing and communicating the expectations of patients and families effectively.
- Evaluating the bereavement and pre-bereavement needs of clients and developing a tailored bereavement care plan based on this assessment.
- Ensuring accurate and timely documentation of services provided.
- Meeting service and quality standards to enhance patient and family satisfaction.
Required Knowledge, Skills & Abilities:
- Practices within the legal scope of license, certification, or registration as applicable.
- Ability to function effectively in diverse care environments, including home settings, skilled nursing facilities, and assisted living communities.
- Adherence to safety and infection control protocols.
- Proficiency in Microsoft Office Suite (Excel, Word, Outlook, PowerPoint) and virtual communication platforms (Zoom, TEAMS).
Qualifications:
- Master of Divinity or an equivalent advanced degree in Theology, Religious Education, Pastoral Counseling, or related fields is preferred.
- Completion of 1-4 units in Clinical Pastoral Education from a recognized provider.
- A minimum of two years of ministerial experience.
- Experience in home health or hospice settings is advantageous.
- A valid driver's license and proof of automobile insurance or alternative personal transportation are required.
- Previous healthcare experience with patients facing serious illnesses is preferred.
