Amedisys, a leading provider of home health, hospice, and high-acuity care, invites skilled and compassionate Registered Nurses (RNs) to join their team as Case Managers. Key Responsibilities:
  • Perform patient assessments and collaborate with the care team to develop and implement a plan of care.
  • Make referrals to other disciplines as indicated by the patient's identified needs or document rationale for not doing so.
  • Teach patients and their families self-care techniques and promote patient health and independence.
  • Supervise Licensed Practical Nurses (LPNs) and Home Health Aides (HHAs).
  • Complete documentation timely and accurately and communicate patient progress to the clinical manager and care team.
Requirements:
  • One (1+) year of clinical experience as a Registered Nurse (RN).
  • Current RN license, specific to the state(s) assigned to work.
  • Current CPR certification.
  • Valid driver's license, reliable transportation, and liability insurance.
