Key Responsibilities- Conduct thorough patient assessments, including history and physical exams, to identify and address health concerns.
- Develop and implement individualized care plans, taking into account patient preferences and needs.
- Collaborate with supervising physicians to ensure comprehensive and coordinated care.
- Provide education and counseling to patients and their families on health-related topics and disease management.
- Document patient care and outcomes in accordance with established guidelines and regulations.
- Participate in quality improvement initiatives and contribute to the development of best practices in patient care.
- Master's degree in Nursing with a focus on Nurse Practitioner education.
- Current licensure as a Registered Nurse and Nurse Practitioner in the state of Texas.
- Board certification as a Nurse Practitioner.
- Minimum one year of experience as a Nurse or Nurse Practitioner.
- Experience working in a primary care setting.
- Strong communication and interpersonal skills.
- Ability to work effectively in a team environment.
This role requires working indoors with occasional exposure to mechanical and electrical hazards. The physical demands of this position are classified as light work.
