RN - Home Health Specialist

We are seeking a skilled RN to join our team as a Home Health Specialist. As a Home Health Nurse, you will play a vital role in providing high-quality patient care and collaborating with healthcare teams to optimize outcomes.
Key Responsibilities:- Provide excellent patient care by assisting in the development, implementation, and revision of the case management program.
- Act as a liaison between patients, families, nurses, physicians, and multidisciplinary teams to ensure seamless care coordination.
- Lead the coordination of patient care with other disciplines within the care team, monitoring the appropriateness and timeliness of care.
- Ensure the interdisciplinary care plan is consistent with the patient's clinical course, continuing care needs, and covered services.
- Collaborate with acute care case managers to ensure appropriateness of ongoing care.
- Coordinate the transfer of patients to tertiary centers, including the transfer of patient information required for continuity of ongoing treatment and services.
- Provide oversight and collect data required for regulatory/accreditation compliance.
- 2+ years of Nursing experience - REQUIRED
- 6 months of Home Health experience - REQUIRED
- Travel experience - REQUIRED
- Active and clear driver's license and car insurance - REQUIRED
- Hawaii license - REQUIRED
- Homecare Home Base and OASIS - REQUIRED
- COVID and FLU vaccine - REQUIRED. Will not take declinations
- Bachelor's degree - preferred
- Case Manager experience - preferred
Mileage reimbursed at.43 cents per mile (lower than IRS established rate)
