Key Responsibilities
  • Provide patient care and services as outlined in the care plan, collaborating with Community Health Nurses and Therapists as needed.
  • Maintain effective communication with patients, families, and healthcare team members to ensure seamless care coordination.
  • Accurately document patient care and progress, adhering to departmental and regulatory requirements.
  • Adhere to departmental and system protocols, HIPAA regulations, and local, state, and federal requirements.
Requirements
  • HHA certification from the New York State Education Department (NYSED)
  • Valid New York State Driver's License (NYS DL)
  • Ability to read, write, and speak English
  • Ability to follow verbal and written instructions
