Key Responsibilities:- Provide occupational therapy services to adult and geriatric patients
- Prepare comprehensive documentation, progress notes, and discharge summaries
- Develop personalized plans of care tailored to patients' needs and capabilities
- Valid State Occupational Therapist License and Credentials
- Experience in SNF and/or Acute care settings
- Covid Vaccine required in certain locations
