About the Job

This role offers the opportunity to provide compassionate care to patients in their homes, assisting with daily living activities, and supporting their emotional and spiritual well-being. You will work under the direction of a Registered Nurse and/or Clinical Supervisor, providing personalized care to meet each patient's unique needs.

Responsibilities
  • Assist patients with maintaining good personal hygiene
  • Help maintain a safe and healthy environment
  • Prepare and serve light meals
  • Assist with feeding patients when needed
  • Assist patients with ambulation
  • Use walkers or wheelchairs safely as taught by nurses
  • Perform light housekeeping tasks, such as emptying trash and changing linens
  • Follow treatments ordered on the aide plan of care and state-specific regulations
  • Encourage patients' mental alertness through engaging activities
  • Offer basic emotional and psychological support to patients and their families
  • Report changes in patients' conditions or home situations to the Registered Nurse and interdisciplinary team
Requirements

To be successful in this role, you should possess a Certified Nursing Assistant (CNA) license and at least six months of patient care experience in hospice, home health, ICU, geriatric, or oncology settings. Additionally, experience in hospital Staff RN, Med Surg, PACU, Float pool, ER, critical care, acute care, cardiac, geriatric, admissions, wound care, telemetry, nursing home, or travel can be beneficial.
