Job Title: Homecare Physical Therapist

We are seeking a compassionate Physical Therapist to provide home-based physical therapy services to our members in Nassau County, NY.

Job Summary:

As a Physical Therapist with Centers Plan for Healthy Living, you will travel to patients' homes to provide physical therapy as ordered by a physician. You will design treatment plans that achieve results through personalized, 1-on-1 individualized care. Your responsibilities will include evaluating effects of treatment, consulting with patients, diagnosing movement dysfunction, and developing quality care intervention programs.

Responsibilities:
  • Travel to and from patients' homes to provide physical therapy services
  • Design treatment plans that achieve results through personalized care
  • Evaluate effects of treatment and adjust treatments as needed
  • Consult with patients to learn about their physical condition and symptoms
  • Diagnose movement dysfunction and develop quality care intervention programs
  • Use hands-on therapy to ease patients' pain and promote healing
  • Maintain patient records and adapt treatment plans accordingly
  • Instruct family members and caregivers in physical therapy techniques
Qualifications:
  • Current Physical Therapy Licensure in NY State
  • Graduate degree or greater in physical therapy
  • Proven work experience as a physical therapist
  • Intensive education and clinical expertise
  • Ability to manage patients with different personalities
  • Current knowledge of treatment practices
  • Excellent interpersonal communication skills
Compensation:

$80.00 per visit
