Job Summary

The Licensed Practical Nurse (LPN) plays a vital role in ensuring the well-being of residents at Convivial Life. Reporting to the Health & Wellness Director, this position requires a skilled professional to provide a system of services that promotes dignity and individuality for each resident.

Essential Functions
  1. Coordinate admission and orientation of new residents, ensuring a smooth transition for the resident and their loved ones.
  2. Monitor and maintain records of residents' health and mental status, reporting any changes to the resident's physician in a timely manner.
  3. Administer treatment and record residents' responses.
  4. Make recommendations regarding medication to the physician and work with the pharmacy to ensure proper handling, dispensing, and destruction of medication.
  5. Ensure that all resident rights are honored and respected by staff members.
  6. Execute infection control duties and ensure staff compliance.
  7. Coordinate with dietary staff to ensure nutritional and therapeutic diets are served to residents as needed.
  8. Investigate and document any resident incidents or issues.
  9. Coordinate resident discharges and transfers.
  10. Observe and assist in direct resident care.
  11. Assist new staff with ongoing training.
  12. Work under potentially stressful conditions with strict deadlines.
  13. Participate in resident programming on a daily basis.
Essential Mental and Physical Functions

The following mental and physical functions are required to perform the essential duties of this position:

  • Ability to read and write, follow written and verbal instructions, and communicate effectively in English.
  • Thinking critically, defined as reading, understanding, and taking appropriate action on detailed and complex information.
  • Walking long distances regularly.
  • Lifting and carrying up to 50 lbs.
  • Pushing a wheelchair with a resident weighing up to 200 lbs.
  • Repetitive actions/motion of one or both hands.
  • Hearing ordinary conversation and office sounds.
  • Verbal communication in person and on the phone.
  • Written communication by hand and on PC.
Qualifications

The ideal candidate will possess the following qualifications:

  • Licensed Florida LPN, required to maintain licensure.
  • Completed LPN Supervisory Course or ability to complete within 6 months of employment.
  • Two or more years of related experience and/or training in geriatrics in long term care, home health, or other healthcare settings.
  • Excellent verbal and written communication skills.
  • Computer literacy, including Microsoft Office suite.
  • Outstanding communication and interpersonal skills.
