Rural Health Care Coordinator

We are seeking a highly skilled and dedicated Rural Health Care Coordinator to join our team at SouthEast Alaska Regional Health Consortium. As a key member of our nursing department, you will play a vital role in coordinating the delivery of high-quality patient care, promoting cost-effective health outcomes, and fostering a collaborative environment among healthcare professionals.
Key Responsibilities- Case Management: Act as a primary point of contact for patients, collaborating with medical providers and interdisciplinary teams to establish individualized care plans that maximize patient health outcomes.
- Clinical Coordination: Coordinate multi-disciplinary care conferences for high-risk or complex patients, ensuring timely and effective care delivery.
- Patient Advocacy: Serve as a patient advocate, working closely with families and caregivers to ensure informed decision-making and seamless care transitions.
- Referral Coordination: Facilitate referrals to specialty clinics, managing waitlists and ensuring timely completion of work-up labs and diagnostics.
- Clinical Support: Provide clinical support for specialty clinics, including assessment, documentation, education, and medication administration.
- Education and Outreach: Develop and implement education programs for patients and families, promoting health literacy and self-management skills.
- Quality Improvement: Participate in quality improvement initiatives, identifying areas for process improvement and implementing evidence-based practices to enhance patient care.
- Education: Graduate of an accredited school of nursing, Bachelor's degree preferred.
- Licensure: Current full, unrestricted Registered Nursing license in Alaska or other U.S. state required.
- Experience: At least 1 year of general nursing experience, with at least 6 months' employment as a clinical nurse at a SEARHC outpatient clinic, or at least 2 years of general nursing experience with a minimum of 1 year in an ambulatory setting or 1 year case management experience.
- Certifications: Basic Life Support certification preferred, National Case Management certification preferred.
