Registered Nurse Care Coordinator

The Registered Nurse Care Coordinator plays a pivotal role in ensuring seamless patient experiences by collaborating with patients, their families, physicians, and the interdisciplinary team. This position is essential in managing and optimizing the patient's pathway through the Emergency Department and acute care phases.
The RN Care Coordinator is responsible for monitoring and enhancing the quality of care from the moment of admission until discharge. Key Responsibilities:
- Implementing established protocols and order sets tailored to the patient's admission diagnosis.
- Guiding the progression of care and assisting patients in navigating through acute care.
- Developing appropriate transition plans within a day of admission.
As a dedicated advocate for patients and their families during their Emergency Department visit and throughout the acute care process, the RN Care Coordinator is committed to delivering the highest quality of care and ensuring a positive experience.
Essential Skills:- Outstanding interpersonal communication and negotiation skills.
- Knowledge of community and healthcare system resources.
- Excellent organizational and time management capabilities.
- Ability to work independently.
- Prior experience with software such as Word, Excel, and Meditech is beneficial.
