Job Title: Licensed Practical Nurse

VITAS Healthcare is seeking a compassionate and skilled Licensed Practical Nurse (LPN/LVN) to join our team of dedicated healthcare professionals. As an LPN/LVN, you will play a vital role in providing high-quality, patient-centered care to individuals with terminal illnesses.

Responsibilities:
  • Administer nursing care according to the plan of care, in accordance with the Nurse Practice Act in the state where the VITAS program is located.
  • Evaluate patient/family needs and assist in the preparation and execution of the interdisciplinary plan of care for each patient on the team.
  • Perform selected acts in the care of the terminally ill under the supervision of a registered nurse, including the administration of treatments and monitoring patient reactions.
  • Report significant findings or changes in the patient's condition to the registered nurse, Team Manager, or Physician, and document reports in clinical notes.
  • Maintain a clean and safe environment for the patient and family, and ensure accurate documentation, including clinical notes on interdisciplinary charts.
  • Evaluate patient/family response to care, and utilize a wide variety of supportive and technical nursing skills in carrying out specific medical orders.
  • Assist the interdisciplinary team in designing and delivering the interdisciplinary plan of care, and establish therapeutic relationships with patients/families to ensure their participation in the plan of care.
  • Initiate and perform nursing protocols based on patient needs, relating to physical comfort and mental well-being, including positioning, mobilizing, bowel and bladder elimination, skin and wound care, personal hygiene, and patient/family teaching.
Requirements:
  • Graduate of an approved school of Practical/Vocational nursing.
  • Currently licensed to practice nursing in the state where the VITAS program is located.
  • At least one year of satisfactory nursing experience, at least one year of which is an LPN/LVN in a hospital on an adult medical surgical unit.
