About the Role

We are seeking a compassionate and dedicated Hospice Registered Nurse to join our team at Heartland Hospice. As a key member of our interdisciplinary team, you will provide high-quality patient care and support to patients and their families during a critical time.

Responsibilities
  • Provide Direct Patient Care: Offer physical, emotional, and spiritual support to patients and their families, ensuring their comfort, dignity, and well-being.
  • Act as Primary Liaison: Facilitate communication between healthcare providers, patients, and families, ensuring seamless care coordination.
  • Develop and Implement Care Plans: Collaborate with the interdisciplinary team to create and execute personalized care plans, addressing patients' unique needs and goals.
  • Manage Patient Records: Maintain accurate and comprehensive clinical records, ensuring timely and efficient documentation.
  • Participate in Quality Initiatives: Contribute to quality improvement efforts, ensuring that our care meets the highest standards.
