About the Role:

We are seeking a skilled Home Health Aide to join our team at Premier Infusion & Healthcare Services, Inc. As a Home Health Aide, you will provide high-quality, personalized care to patients in their homes, promoting independence and dignity.

Responsibilities:

  • Provide hands-on personal care, including bathing, back rubs, oral hygiene, shampoos, and changing bed linen.
  • Assist patients with dressing and undressing, skin care to prevent breakdown, and toileting activities.
  • Keep patient living areas clean and orderly, as appropriate, and plan and prepare nutritious meals.
  • Assist in feeding patients, if necessary, and take and record oral, rectal, and axillary temperatures, pulse, respiration, and blood pressure when ordered.
  • Assist in ambulation and exercise according to the plan of care and perform range of motion and other simple procedures as an extensional therapy service as ordered.
  • Perform simple procedures as an extension of nursing services as ordered and assist patients in the self-administration of medication.
  • Do patient laundry, as appropriate, and meet safety needs of patients, using equipment safely and properly.
  • Report on patient condition and significant changes to the assigned nurse and adhere to the Organization's documentation and care procedures and standards of personal and professional conduct.

Requirements:

  • Meets the training requirements and successfully completed a training and competency evaluation program that meets § b) and (c); or a competency evaluation that meets the requirements of § c); or a nurse aide training and competency evaluation approved by the state as meeting the requirements of § through § and is currently in good standing on the state nurse aide registry; or at least 18 years of age.
  • Excellent observation, verbal, and written communication skills, problem-solving skills, basic math skills, and document care given.
  • Self-directing with the ability to work with little direct supervision, empathy for the needs of the ill, injured, frail, and impaired, and demonstrates tact, patience, and good personal hygiene.
  • Prolonged or considerable walking or standing, able to lift, position, or transfer patients, and able to lift supplies and equipment.
  • Considerable reaching, stooping, bending, kneeling, or crouching, visual acuity, and hearing to perform required nursing skills.
  • Licensed driver with an automobile that is insured in accordance with Organization requirements and is in good working order.
