- Coordinate Patient Care: Serve as a primary point of contact for patients, families, and healthcare teams, ensuring well-coordinated care and timely communication.
- Execute Physician and Nursing Orders: Accurately and efficiently carry out physician and nursing orders, prioritizing patient safety and well-being.
- Assess and Coordinate Discharge Planning: Work closely with patients, families, and healthcare teams to identify and address discharge planning needs, ensuring a smooth transition to post-acute care.
- Provide Patient Education: Assess learning needs, develop educational plans, and deliver patient-centered education to promote health literacy and self-management.
- Comply with Nursing Practice Act: Adhere to the Nurse Practice Act, ensuring that patient care is delivered in accordance with established standards and guidelines.
- Active Kentucky Registered Nurse or LPN Licensure: Possess current licensure to practice as a Registered Nurse or Licensed Practical Nurse in the state of Kentucky.
- BLS and ACLS Certification: Hold current Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS) certifications.
