We are seeking a skilled Physical Therapist Assistant to join our team at Home Recovery-Home Aid. As a Physical Therapist Assistant, you will play a vital role in providing high-quality patient care and contributing to the success of our home healthcare agency.
Key Responsibilities- Observation and Reporting: Observe and report any deviations in patients' health status, utilizing age-appropriate norms and principles of growth and development.
- Physical Therapy Services: Provide physical therapy services to patients in their homes, following the Plan of Care established by the physician and under the direction of the Physical Therapist.
- Client Education: Identify unmet client needs and report to the Physical Therapist and the VP of Clinical Services or Director of Professional Services. Provide education and instruction to clients and caregivers to enhance their self-care abilities.
- Vital Sign Monitoring: Monitor vital signs as necessary, in accordance with the patient's condition and treatment plan.
- Treatment and Documentation: Provide physical therapy treatments to relieve pain, develop or restore function, and maintain maximum performance, in accordance with physicians' orders and under supervision of a licensed Physical Therapist. Record and report any changes in the patient's condition or reaction to treatment.
- Collaboration and Communication: Instruct clients, family members, or other healthcare personnel in the patient's physical therapy program, including the use of prosthetic, orthopedic, and assistive devices. Participate in case conferences and discharge planning.
- Record Keeping: Maintain accurate and timely records and reports, in accordance with Agency policies and procedures.
- Education and Licensure: Graduation from a two-year, college-level program approved by the American Physical Therapy Association. Current Physical Therapy Assistant license in MD.
- Experience: One year of home health experience preferred. Current CPR certification. Excellent communication, organizational, and human relations skills.
- Skills and Abilities: Ability to function with minimal supervision, manage multiple projects and changing priorities, and employ critical thinking skills.
