**Job Summary:**

As a Licensed Vocational Nurse with La Casa Health Care, you will have the opportunity to make a meaningful difference in the lives of our patients and their families. Your primary responsibility will be to provide skilled nursing care to patients in their place of residence, including administering medications, wound care, and other treatments as assigned.

**Responsibilities:**

* Implement the nursing care plan for each patient
* Provide nursing services, treatments, and diagnostic and preventive procedures as assigned
* Initiate preventive and rehabilitative nursing procedures as appropriate
* Observe signs and symptoms and report to the physician and RN reactions to treatments, including drugs and changes in the patient's physical or emotional condition
* Teach and counsel the patient and family/significant others regarding the nursing care needs and other related problems of the patient at home
* Prepare clinical and progress notes
