We are seeking a skilled Nurse Practitioner to join our Clinical team at EasyHealth. As a key member of our team, you will conduct comprehensive health risk assessments for Medicare, Medicaid, and Commercial Marketplace members/patients of primary care physicians, medical groups, and the health plans we serve.
Responsibilities- Complete detailed evaluations of the medical and social assessment of beneficiary members.
- Identify and refer members/patients for routine, urgent, and emergent case management.
- Make recommendations for members' follow-up with their primary care physician.
- Conduct comprehensive assessments face-to-face in the home setting.
- 2 years of hands-on experience as a Nurse Practitioner.
- Registered with Medicare and Medicaid.
- Current, unrestricted license in the state of assignment.
- License/certification: APRN, Certified Nurse Practitioner.
- Knowledge of the HCC model, HEDIS, and STAR quality measures.
- Effective communication with elderly and chronically ill members.
- Proficient in the use of computer, tablet, and mobile technology.
- Ability to document within available EMR.
- Independent Contractor
- Flexible schedule & part-time positions available
- 1-hour session per visit
- Compensation: $100 per completed visit
- Sign-on bonus of $500
