Overview
GEO Reentry Services, LLC is seeking a highly motivated and dedicated Case Manager to join our team. As a Case Manager, you will play a critical role in supporting residents in developing program plans and administering policies and procedures.
Responsibilities
- Manage a caseload of client cases and provide support and structure to residents in developing program plans.
- Administer policies and procedures of the program as directed by the contracting agency and the Facility Director.
- Learn, use, and model the principles of the 'What Works' system through evidence-based practices in all interactions with clients and other staff members.
- Train and assist less experienced case managers.
- Perform duties of a Monitor, including resident intakes, check-ins, breath analysis, and drug screens, as needed.
- Prepare written personalized programs for each resident, establishing measurable criteria of expected behavior and accomplishments, and a time frame for achieving specified goals.
- Review plans with residents on a weekly basis and document progress or revisions.
- Assist residents in establishing personal budgets, locating suitable housing, enrolling in educational and vocational programs, and participating in family and community activities.
- Monitor facility sanitation and safety, assigning daily housekeeping chores to residents as needed.
- Orient residents upon arrival at the facility, providing a smooth assimilation into the program.
- Make entries into confidential resident files, providing structure for case analysis and program planning, and documenting program participation.
- Ensure files are secure to protect resident confidentiality.
- Assist the Employment Placement Specialist in verifying residents' jobs, ensuring employment is suitable under the established program plan.
- Counsel unemployed residents, providing guidance in obtaining employment in cooperation with the Employment Placement Specialist.
- Verify residents' pass plans through pass sponsorship, ensuring residents are accountable for their whereabouts in the community.
- Prepare terminal reports upon resident release, summarizing program performance and satisfying contract requirements.
- Participate in staff meetings, sharing observations and recommendations regarding the facility, residents, and staff relations.
- Maintain confidentiality obtained through job duties, providing sensitive information only on a 'need-to-know' basis.
- Monitor client compliance with alcohol and drug counseling when required.
- Report program violations to the appropriate agency or official via established reporting parameters.
- Conduct orientation meetings with new clients, explaining program rules and regulations, and verbally explaining the client contract in detail.
- Install electronic monitoring equipment on new clients as necessary.
- Set client schedules and specify approved activities based on sentencing information.
- Approve temporary schedule changes for approved activities.
- Communicate effectively with all levels of staff, promoting a team spirit and sense of cooperation.
- Comply with work rules and policies, responding positively to directives from managers and supervisors.
- Adhere to attendance policy, working overtime as required to ensure adequate coverage of the program and resident safety.
- Comply with safety rules, taking corrective action to ensure work is performed in a safe manner and without injury to self or others.
Qualifications
- Bachelor's Degree in social, human services, criminal justice, or behavioral science program from an accredited college or university.
- Minimum of one year of experience working in human services, corrections, or community-based services.
- Work experience may be substituted for academic studies, exchanging one year of work experience in a related field for one year of academic education.
- Familiarity with community corrections systems and current community corrections practices.
- Familiarity with community workforce needs, public and private agencies, and employment services.
- Ability to write clear, concise reports in English using correct grammar.
- Ability to add, subtract, multiply, divide, and calculate percentages.
- Must be able to react quickly and appropriately in an emergency situation.
- Bilingual in English/Spanish preferred but not mandatory.
- Ability to work with computers and necessary software typically used by the department.
