Key Responsibilities:- Communicate effectively with patients and providers to identify, prevent, and solve drug-related problems.
- Provide accurate and timely information to healthcare providers and make appropriate therapy recommendations.
- Document actions and results in patients' electronic medical records and pharmacy software system.
- Conduct final review of prescriptions to ensure accuracy and completeness.
- Identify and provide counseling to patients based on current prescriptions and medication history.
- Provide medication counseling to patients who desire to speak with a pharmacist in person or on the phone.
- Teach patients how to use insulin and other injectable products, inhalers, glucometers, and other medical devices.
- Oversee the work of pharmacy technicians and play an active role in technician training.
- Inventories and documents controlled substances in accordance with LCHC policies.
- Process returns on the cash register when necessary and appropriate.
- Help maintain security of the pharmacy department.
- Report medication errors in accordance with LCHC policies.
- Participate in the pharmacy and therapeutics committee at the discretion of the Director of Pharmacy.
- Attend staff meetings and provider meetings.
- Participate in performance evaluations and quality improvement efforts.
- Maintain own physical and mental health required to perform duties, including required current immunizations.
- Maintain professional competence by pursuing opportunities for continuous learning and complete required continuing pharmacy education requirements.
- Comply with all state and federal pharmacy practice laws, HIPAA, OSHA, and JCAHO regulations and pharmacy policies and procedures.
- Doctor of pharmacy (PharmD) degree from an accredited institution.
- Registered pharmacist in good standing in the state of Illinois.
- Spanish language skills are helpful.
- OIG background check performed prior to employment.
