We are seeking a highly skilled Licensed Mental Health Professional to join our team at Fairview Health Services. As a key member of our interdisciplinary team, you will provide onsite Mental Health Diagnostic Assessments to individuals of all ages at our MHealth Fairview Riverside location.
Responsibilities:- Provide coordinated care by initiating client-centered consultation with internal and external providers involved in care or services.
- Provide crisis call support and determine an appropriate level of care, as needed, for clients.
- Complete a comprehensive behavioral assessment to identify risk, risk mitigation strategies, other patient needs, and to recommend a level of care and disposition plan.
- Conduct diagnostic evaluation and document assessment in medical record.
- Develop an individualized treatment plan of care.
- Collaborate with patient, family, and other colleagues to develop a treatment plan of care.
- Maintain required, appropriate, and timely communication and correspondence with treatment authorization and payer sources in a manner which promotes efficient and effective delivery of clinical services.
- Complete the DEC (Diagnostic Evaluation Center) assessment form for ED patients.
- Determine appropriate disposition, coordinates care, and provides discharge safety and aftercare plans for patients.
- Collaborate with the patient, interdisciplinary team, and family to create a treatment plan based on medical necessity and the patient's preference for care.
- Provide primary communication to collaborating partners, e.g. hospital emergency departments, schools, primary care clinics, assisted living, group homes, and other treatment providers.
- Master's Degree, from an accredited school, in Social Work, Psychology, or closely related discipline.
- Two or more years' experience in crisis counseling/therapy situations.
- Experience serving patients with Mental Health, Substance Use, and Co-Occurring disorders.
- One of the following licenses in good standing in the State of Minnesota: Licensed Psychologist (Doctoral or Master's), Licensed Independent Clinical Social Worker, Licensed Marriage Family Therapist (Doctoral or Master's), or Licensed Professional Clinical Counselor (Doctoral or Master's).
- Three or more years' experience in crisis counseling/therapy.
- Two or more years of working within an Interdisciplinary team in mental health care.
- Access to reliable and timely transportation to any sites as required by role.
- Basic Life Support Certification.
- Good working knowledge of electronic health records as well as a comfort level and willingness to utilize computer-based platforms for assessments along with proficiency with computers required.
- Ability and willingness to provide care at non-primary worksite through face-to-face or telemedicine evaluations.
- Depending on area of work, this position requires after-hours rotation and may be scheduled as determined by needs and requirements of the on-call team.
