About the Position:
We are seeking a skilled Social Worker I to join our team at Willow Creek Behavioral Health, dedicated to delivering exceptional mental health services.
Key Responsibilities:
- Deliver high-quality individual and group therapy sessions to address various mental health concerns.
- Conduct comprehensive assessments of patients' needs, skills, and limitations to develop tailored treatment plans.
- Cultivate strong relationships with multidisciplinary teams to ensure seamless care coordination.
- Communicate effectively with family members to foster understanding and support of the patient's treatment plan.
- Maintain accurate and timely documentation of patient records, adhering to facility and accreditation standards.
- Develop and implement evidence-based treatment plans that prioritize patient safety and well-being.
- Requirements:
- Bachelor's degree in Social Work from an accredited institution.
- Active pursuit of a master's degree in social work.
- One or more years of experience working with the patient population preferred.
- Eligibility for state licensure as a Licensed Social Worker (LSW), Licensed Associate Counselor (LAC), Certified Addictions Counselor (CAC), or Registered Addiction Counselor (RAC).
