Care Manager for Health Services

Sea Mar Community Health Centers is a leading provider of quality healthcare services, and we are seeking a skilled Case Manager to join our team.

The Case Manager will be responsible for providing support to patients and families, and technical or caseload support to the care management team.

The ideal candidate will have a strong understanding of common chronic conditions and the emotional issues that can come from these conditions.

The Case Manager will assist patients in achieving their self-management goals, improving their healthcare experience, and enhancing their overall well-being.

Key Responsibilities:

  • Provide support to patients and families
  • Assist with skill building and achieving small steps toward goals
  • Meet with patients in their home, community, or clinic
  • Assist with groups such as Living Well with Chronic Conditions
  • Advocate for patients with chronic conditions

Requirements:

  • High School Graduate or equivalent
  • One year paid or volunteer experience in case management or care coordination
  • Background in community outreach or case management preferred
  • CPR certification and valid driver's license required
