POSITION SUMMARY: Under the supervision of a Registered Nurse (RN), the Licensed Practical Nurse (LPN) provides effective and efficient nursing care, ensuring positive clinical outcomes and patient satisfaction. The LPN operates within the scope of practice as defined by the State Nurse Practice Act. Responsibilities include contributing to nursing assessments, care planning, delivering direct patient care, and overseeing care provided by unlicensed personnel.

RESPONSIBILITIES/ACCOUNTABILITIES:

1. Patient Evaluation:
1.1. Gathers, reports, and documents both objective and subjective data;
1.2. Monitors patient conditions and communicates changes to the RN;
2. Care Planning:
2.1. Aids in setting individualized patient goals;
2.2. Assists in formulating interventions to meet those goals;
2.3. Executes the care plan;
2.4. Assesses the effectiveness of interventions to achieve patient goals and reduce rehospitalizations;
2.5. Engages in the review and modification of the care plan;
3. Provision of Direct Patient Care:
3.1. Administers medications and performs treatments as per physician directives;
3.2. Relays pertinent information to the RN and/or physician;
3.3. Maintains accurate and comprehensive documentation;
3.4. Seeks guidance from the RN as necessary;
3.5. At the RN's direction, participates in Utilization Management or Interdisciplinary Care Management Meetings to facilitate appropriate discharge planning;
4. Oversight of Unlicensed Staff:
4.1. Assigns duties in accordance with the State Nurse Practice Act and organizational policies;
4.2. Shares patient information with unlicensed staff during and between shifts;
4.3. Monitors and evaluates the responsibilities of unlicensed staff;
4.4. Ensures compliance with policies and procedures;
5. Utilizes Point Click Care (PCC) in line with business processes;
6. Conducts drug counts at the start and end of shifts and as needed;
7. Assists physicians during rounds and patient examinations as required;
8. Engages in shift-to-shift communication between nursing staff;
9. Collaborates with other departments to deliver timely and effective care tailored to individual needs;
10. Fosters a respectful, team-oriented environment responsive to the concerns of staff, patients, and families;
11. Enhances nursing practice by attending mandatory in-service programs;
12. Aids in the orientation of newly hired nursing personnel;
13. Promotes a culture of safety to ensure a healthy practice environment;
14. Maintains confidentiality and safeguards sensitive Protected Health Information (PHI);
15. Stays beyond scheduled shifts if necessary to meet staffing requirements;
16. Performs additional duties as assigned.

COMPLIANCE:
1. Adheres to applicable legal requirements, standards, policies, and procedures;
2. Participates in required orientation and training programs;
3. Reports concerns and suspected compliance issues to supervisors;
4. Cooperates with monitoring and audit functions;
5. Engages in quality assurance and process improvement activities as requested.

SPECIFIC EDUCATIONAL/VOCATIONAL REQUIREMENTS:
1. Graduation from an accredited practical or vocational nursing program is required;
2. Current licensure by the State Board of Nursing is mandatory;
3. Experience in rehabilitative or geriatric nursing is preferred;
4. Proficiency in reading, writing, speaking, and understanding English is essential for patient safety;
5. Verification of TST (tuberculin skin test) is required;
6. Current BLS/CPR certification is necessary.
