Job Summary:
The successful candidate will be responsible for providing high-quality primary care services to patients of all ages, from pediatric to geriatric. The ideal candidate will have a strong background in chronic disease management and be able to work effectively in a fast-paced environment.
Key Responsibilities:
- Provide comprehensive primary care services to patients, including routine check-ups, vaccinations, and health screenings.
- Manage chronic diseases, such as diabetes and hypertension, and develop treatment plans to improve patient outcomes.
- Collaborate with a team of healthcare professionals, including RNs and medical assistants, to provide seamless patient care.
- Conduct patient assessments, diagnose conditions, and develop treatment plans.
- Order and interpret diagnostic tests, such as lab results and imaging studies.
- Prescribe medications and treatments as needed.
Requirements:
- Master's degree in Nursing or related field.
- Current licensure as a Family Nurse Practitioner.
- Minimum 2 years of experience in primary care.
- Strong background in chronic disease management.
- Excellent communication and interpersonal skills.
