About the RoleThis is a full-time opportunity to work in a skilled nursing facility, providing primary care to patients in a post-acute setting. As a nurse practitioner, you will have the autonomy to manage chronic disease, diagnose illnesses, and implement plans of care. You will also have the opportunity to work collaboratively with a multidisciplinary team, including nurses, therapists, and social workers.
Key Responsibilities- Provide primary care to patients in a post-acute setting
- Manage chronic disease and diagnose illnesses
- Implement plans of care and coordinate patient care with other healthcare practitioners
- Educate patients and their caregivers on coping skills and healthy lifestyle changes
- Build relationships with patients, families, and facility staff to facilitate effective treatment delivery
- A minimum of 1 year experience as a nurse practitioner in a primary care setting
- Active and unrestricted Washington NP license
- Active and unrestricted Washington DEA
