POSITION SUMMARY: The Licensed Practical Nurse (LPN) plays a crucial role in delivering direct nursing care to residents while overseeing the daily nursing operations conducted by nursing assistants. This supervision must align with current federal, state, and local standards, guidelines, and regulations governing our facility, as directed by the Director of Nursing, to ensure the highest quality of care is consistently upheld.

KEY RESPONSIBILITIES:
  • Oversee the daily activities of nursing assistants. Collaborate with assigned nursing staff and support personnel to plan the services, programs, and activities for the shift. Prepare written and oral reports or recommendations regarding shift activities as necessary. Manage the admission, transfer, and discharge processes for residents. Provide leadership to nursing staff assigned to your unit or shift.
  • Complete and maintain necessary record-keeping forms and charts upon resident admission, transfer, or discharge. Accurately receive and transcribe physician orders, documenting them on the Physician's Order Form. Maintain detailed and informative nurse's notes that reflect the care provided and the resident's response. Complete accident and incident reports and submit them to the Director as required. Perform routine charting duties in accordance with established policies and procedures.
  • Administer medications as prescribed by the physician.
  • Foster and maintain effective working relationships with inter-departmental personnel and other facility departments to ensure nursing services meet resident needs. Create a warm, personal, and positive atmosphere while maintaining a calm environment throughout the unit and shift. Exchange nursing reports at the beginning and end of each shift. Accompany physicians on rounds as necessary.
  • Provide professional services including catheterization, tube feedings, suctioning, dressing changes, colostomy care, and other required procedures.
  • Monitor nursing staff to ensure compliance with established safety regulations regarding equipment and supplies.
  • Uphold the confidentiality of all resident care information, including protected health information. Report any known or suspected breaches of confidentiality. Ensure that all nursing care is delivered in privacy, and that nursing personnel knock before entering resident rooms.
  • Perform additional duties as assigned.
QUALIFICATIONS: To be successful in this role, candidates must meet the following requirements:
  • Current and unencumbered state license as a Licensed Practical Nurse.
  • Proficient in reading, writing, speaking, and understanding the English language.
  • Able to make independent decisions when necessary.
  • Knowledgeable about nursing and medical practices, procedures, and relevant laws and regulations.
  • Current CPR Certification is mandatory.
  • Experience in Long Term Care is preferred.
PHYSICAL REQUIREMENTS: The physical demands of this position include:
  • Ability to move intermittently throughout the workday.
  • Capacity to manage the mental and emotional stress associated with the role.
  • Ability to function independently and work effectively with residents, staff, and support agencies.
  • Must be able to push, pull, move, and/or lift a minimum of 25 pounds to a height of at least 5 feet and carry such weight a minimum distance of 50 feet.
