Job Summary

The Certified Diabetes Educator - Patient Education Specialist will be responsible for directing, designing, implementing, and documenting patient education for Cooper Diabetes Education Center's Self-Management Education Program in Cherry Hill and Willingboro.

This role requires a strong background in diabetes care and education, as well as excellent communication and interpersonal skills. The ideal candidate will be able to work effectively with patients, families, and healthcare teams to provide personalized education and support.

Responsibilities

  • Develop and implement patient education programs for individuals with diabetes.
  • Provide individualized education and support to patients and their families.
  • Collaborate with healthcare teams to ensure seamless care coordination.
  • Document patient education and outcomes in accordance with program requirements.
Requirements

To be considered for this role, you will need:

  • 2 years' experience in diabetes care and education/counseling of patients with Diabetes.
  • RN or MS RD license or PharmD. Masters preferred.
  • NJ-RN or CDCES license or within 36 months of eligibility of CDCES exam.
