The Salvation Army Family Haven Shelter in St. Louis, Missouri, is seeking a Warming Center Case Manager to provide case management services to individuals and families involved in the Warming Center program. The successful candidate will develop case plans to increase clients' functioning and resettle clients in permanent housing, while participating in all phases of case management to advance these plans.
Key Responsibilities- Conduct initial intake assessments of newly admitted residents to determine client needs.
- Develop, implement, and monitor individual case management plans to help individuals achieve the highest level of self-sufficiency.
- Meet with each individual weekly to review progress on case plans and address concerns.
- Provide information and referrals to clients on employment, housing, utilities, budgeting, personal wellness, and parenting to improve client functioning and eliminate barriers to permanent housing.
- Network with community agencies and organizations to enhance relationships and improve client services and referral systems.
The ideal candidate will have a Bachelor's degree in a human services field and three years of related work experience. A combination of education and experience will be considered. The successful candidate will possess an outgoing personality, good communication skills, and the ability to work with diverse cultures. Strong leadership and organizational skills are required, as well as a commitment to carrying out The Salvation Army's mission. Knowledge of The Salvation Army and government, state, and community resources is helpful. The candidate must have working knowledge of computers and reliable transportation, a valid driver's license, and pass a TSA MVR check.
