**Key Responsibilities:**

  • Perform vital signs and screenings as requested by prescribers to ensure accurate patient data.
  • Conduct weekly medication inventory and storage updates, and quarterly inventory reports with RN assistance to maintain a safe and efficient medication management system.
  • Process refill requests from pharmacies and perform blood/urine collections as needed to support patient care.
  • Request and follow up on lab results, and call in prescriptions and refill medications as directed to ensure timely patient care.
  • Manage written prescription documentation and medication injections to maintain accurate patient records.
  • Coordinate patient assistance for medications and complete prior authorizations for Medicare/Medicaid/insurance to ensure patients receive necessary care.
  • Respond to phone calls and emails regarding medications and notify prescribers of adverse reactions to ensure patient safety.
  • Handle requests for records and discharge summaries, and perform metabolic screenings for patients to support patient care.
  • Administer TB testing for staff and communicate information effectively and accurately to ensure a safe work environment.
  • Ensure program operations align with agency certification standards and strategic plan to maintain a high-quality patient care environment.
  • Adhere to program and agency policies and procedures to ensure compliance and maintain a safe and efficient work environment.
