Key Responsibilities- Deliver post-acute care to residents, monitoring their physical and mental wellbeing and tracking vital signs.
- Perform intakes of residents, documenting their health concerns and medical histories.
- Review medical records and charts, ensuring accurate and up-to-date information.
- Administer medications, verifying dosages and ensuring patient safety.
- Maintain a safe and comfortable environment for residents, adhering to care plans and protocols.
- Assist medical staff with orientation and training, ensuring a smooth transition for new residents.
- Current Pennsylvania state license to practice as a Licensed Practical Nurse (LPN).
- Completion of an approved Licensed Practical Nurse (LPN) educational program.
- High School diploma or equivalent.
- Minimum of 6 months of experience as an LPN or in a related role.
- Competitive hourly pay range: $33-$76.
