Job Description

Job Summary:

Longevity Health is seeking an Advanced Practice Provider - Nurse Practitioner or Physician Assistant to join our team. As an Advanced Practice Provider, you will be responsible for providing high-quality, patient-centered care to our members in a variety of settings, including nursing homes and other long-term care facilities.

Key Responsibilities:

  • Provide primary care services to a panel of Longevity Health members, including assessing, diagnosing, and treating medical conditions.
  • Develop and implement individualized care plans to meet the unique needs of each member.
  • Collaborate with other healthcare professionals, including physicians, nurses, and social workers, to ensure comprehensive care.
  • Communicate effectively with members, families, and caregivers to ensure that their needs are met and that they are informed about their care.
  • Participate in quality improvement initiatives to ensure that care is delivered in a safe and effective manner.

Requirements:

  • Master's degree in Nursing or a related field.
  • Current licensure as a Nurse Practitioner or Physician Assistant in the state of practice.
  • Board certification in a relevant specialty.
  • Experience working in a long-term care setting, preferably in a nursing home or assisted living facility.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
