Job Summary: We are seeking a skilled Speech Therapist to join our team at a local Skilled Nursing Facility. As a Speech Therapist, you will work collaboratively with the therapy teams and Rehab Department to provide high-quality speech therapy services to our residents.
Responsibilities:- Conduct comprehensive evaluations and assessments to identify residents' speech and language needs.
- Develop and implement individualized treatment plans to address residents' speech and language goals.
- Provide speech therapy services, including individual and group sessions, to residents with varying levels of speech and language abilities.
- Collaborate with the therapy teams and Rehab Department to ensure seamless care coordination and communication.
- Manage workflow efficiently, including documentation of individual and group treatments, and notes in the electronic records system.
- Familiarize yourself with PDPM (Patient-Driven Payment Model) and align therapy treatments and plans with Medicaid and Medicare billing requirements.
- New Jersey SLP License
- Background and experience in long-term care, specifically in a Skilled Nursing Facility
- Strong organizational, interpersonal, and communication skills
- Ability to manage patients with different types of personalities and ability levels
- Creative problem-solving skills
