Job Description:

  • Provide one-on-one therapy care for patients in their home setting
Primary Job Responsibilities:
  • Assess a person's condition and develop individualized plans of care to help people reach their goals and maximize mobility and independence
  • Collaborate with an interdisciplinary team and colleagues to ensure a person's needs are met
  • Demonstrate ability to perform standard therapy measurement techniques, including ROM, strength, and balance tests
  • Utilize a variety of therapeutic modalities to maximize care provided to clients
Requirements:
  • Vermont PT licensure
  • Home Health experience and OASIS experience required
  • McKesson/Netsmart EMR experience preferred
